Couch Spaceship is a referral service that connects individuals with independent, licensed physicians who evaluate patients and, where appropriate, prescribe at-home sublingual ketamine therapy. We are not a medical provider. We do not employ physicians or dispense medication.

Information We Collect

When you submit a new patient inquiry, we collect the contact information you provide — such as your name, email address, and phone number. This information is submitted through a HIPAA-compliant form provided by Hipaatizer and is transmitted securely.

We do not collect payment information. We do not use tracking cookies or third-party advertising technologies.

How We Use Your Information

We use the information you submit solely to follow up on your inquiry and, if appropriate, refer you to an independent physician in our network. We do not sell, rent, or share your information with any third party for marketing purposes.

HIPAA

Our patient intake form is processed through Hipaatizer, a HIPAA-compliant platform. Any health-related information you share through that form is handled in accordance with HIPAA requirements.

Once you are referred to an independent physician, that physician's own privacy practices govern how your clinical information is collected and stored. We recommend reviewing their notice of privacy practices at the time of your consultation.

Data Retention

We retain your contact information only as long as necessary to respond to your inquiry and facilitate a referral. If you would like your information removed, contact us and we will delete it promptly.
